ABSTRACT

Objective:To investigate the perspective of family doctor teams on the status quo of home care for community-dwelling elderly.Methods:The focus group interviews on the status quo of home care for community-dwelling elderly were conducted from August 19 to December 28, 2021. One urban area and 1 suburban area of Beijing was selected by purpose sampling method as target areas. Five community health service centers were selected from each area, and 5 family doctor team members were selected as interviewees in each center. The outline of interviews was as follows: what kinds of home care does the family doctor team provide for the community-dwelling elderly; how does the family doctor team evaluate the existing community-based home care; what suggestions on developing community-based home care for the elderly in the future. The data of the interview were analyzed by the thematic framework.Results:Xicheng district and Miyun district were selected as the study sites in the urban and suburban areas of Beijing, and 41 family doctor team members were interviewed. Three themes and 6 sub-themes were extracted. The study showed that: (1) the family doctor contracted service for the elderly was carried out smoothly in the community; (2) the current situation of home care for the elderly in the community varied, the chronic disease management for the elderly was effective, the health education in the community was popular, the implementation of free annual health check for the elderly in urban areas was more difficult than that in suburban areas, and the family doctor team provided home visits as needed and required; (3) the members of family doctor team usually undertook multiple tasks due to the insufficiency of auxiliary staff, and the incentive mechanism of the home visit was not perfect, which needed to be improved.Conclusion:From the perspective of family doctor teams, the contract service for the elderly is generally carried out smoothly in Beijing communities, but it varies in different types of home care for the elderly, and the work and incentive mechanism of family doctor team need to be improved.

ABSTRACT

Objective:To survey the experiences and perception of caregivers on home care service for community-dwelling elderly.Methods:From August to December 2021, individual semi-structured in-depth interviews were conducted with caregivers of the elderly from five community health centers in Beijing Xicheng and Miyun districts selected by purposive sampling.Results:Nine caregivers,2 males and 7 females aged 49-76 (62.8±9.4) years were interviewed in this study. The nursing care period was 3-48 (17±13) years. Four themes were extracted from the interview data: older people′s own condition was the primary factor influencing the home care needs; medical needs for home care were determined by the caregivers′ factors; community health service was important for meet home medical needs; and supporting of home care services for the elderly need to be strengthened.Conclusion:The experience of elderly home caregivers in using community home medical services is affected by many factors, and in the future, the content of home medical services should be improved, and the skills training of community elderly home caregivers should be strengthened, so as to improve the quality of life at home and promote the health literacy of the elderly.

ABSTRACT

Objective:To survey the working status of outpatient nurses in general practice departments of general hospitals.Methods:From March to April 2021, personal in-depth interviews were conducted with outpatient nurses from general practice residency training bases in Beijing. The thematic framework analysis method was used to analyze the interview data and refine the themes.Results:Fourteen nurses working in general practice outpatient clinics were interviewed in this study. Four themes were extracted from the interview data: (1) inadequate staffing of full-time outpatient nurses in general practice departments of comprehensive hospitals; (2) unclear job functions for outpatient nurses in general practice departments; (3) no standardized patient health education in the department; (4) no relevant training received systematically.Conclusion:General practice departments in general hospitals should setup full-time general practice nurse positions, clarify the job responsibilities, strengthen the relevant training and enhance core competency for nurses working in general practice department.

ABSTRACT

With the rapid population aging in China, healthy elderly care has become a consensus of the whole society. As an important part of healthcare system, community health service plays a vital role in health maintenance and promotion for the elderly, and it is an extremely important foundation for realizing healthy endowment. In this article we summarize the status quo of home-based care for the aged, in which the community health service plays an important role, and also make suggestions for further improvement.

ABSTRACT

AIM:To observe the clinical effect of ranibizumab combined with 577nm micropulse laser in the treatment of severe diabetic macular edema(DME). METHODS:There were 52 eyes of 52 patients diagnosed with severe DME who admitted to the People's Hospital of Guangxi Zhuang Autonomous Region from June 2016 to September 2019. The patients were randomly divided into the observation group(26 patients with 26 eyes, treated with ranibizumab combined with 577nm micropulse laser)and the control group(26 patients with 26 eyes, treated with ranibizumab alone). Patients in both groups received intravitreal injection of ranibizumab with “3+PRN” regimen. Followed up at 9mo after treatment to observe the central macular thickness(CMT), the best corrected visual acuity(BCVA)and the times of intravitreal injection of ranibizumab in the two groups.RESULTS:Compared with before treatment, the CMT and BCVA of the two groups were significantly improved at each time point after treatment(all P<0.001), but there was no difference between the two groups(P>0.05). During the follow-up period, the times of vitreous injection of ranibizumabin the observation group was significantly less than that in the control group(5.88±1.24 times vs 7.12±1.24 times, P=0.001). CONCLUSION:Both ranibizumab combined with 577nm micropulse laser and ranibizumab alone are effective in reducing edema and improving vision in patients with severe DME, but the combination therapy reduces the times of injection.

ABSTRACT

Objective:To apply Star Excursion Balance Test (SEBT) in evaluating dynamic balance function in stroke patients. Methods:From September, 2018 to November, 2019, 38 stroke hemiplegic patients of Brunnstrom IV and above who could walk independently were included. They completed SEBT by two testers; the anterior, posteromedial and posterolateral extension standardized distance of the affected leg support were recorded. One of the testers measured SEBT again the day after the next day. The intraclass correlation coefficient (ICC) was calculated. The limits of stability on different directions were measured with active balancer EAB-100 (EAB) and tested with Timed 'Up & Go' Test (TUGT) by the third tester. The correlation of SEBT to EAB and TUGT was analyzed with Pearson correlation coefficient. Results:ICCs of three directions was 0.892 to 0.951 between testers, and 0.888 to 0.963 of test-retest. The standardized distance of three directions of SEBT correlated with the anterior and lateral stability limits of EAB (r = 0.479 to 0.671, P < 0.05), as well as TUGT (r = -0.557~-0.633, P < 0.05). Conclusion:SEBT is valid and reliable for evaluation of the dynamic balance for stroke patients of Brunnstrom IV and above who can walk independently.

ABSTRACT

This study was designed to compare normal and diabetic rats in the pharmacokinetic and tissue distribution of honokiol and its metabolites. Type 2 diabetic rat model was established using high-fat diet feeding for 6 weeks followed by single intraperitoneal injection of low dose (30 mg·kg-1) strepotozotocin. The concentration of honokiol and its metabolites were determined by high performance liquid chromatography. Pharmacokinetic parameters were calculated using DAS 3.0 software. The Cmax of honokiol in the normal rats and diabetic rats are 872.5 ±233.1 and 614.7 ±182.4 μg·L-1 respectively (P -1 respectively. Meanwhile, AUC0-t and Cmax of the metabolite M2 were significantly increased in diabetic rats. The concentrations of honokiol in liver, kidney and brain in the diabetic model group were higher than those of the normal control group. Meanwhile, the exposure of M1 in liver, M2 in liver and kidney of the model group were higher than that in the normal group. The data suggest that the pharmacokinetics and tissue distribution of honokiol and its metabolites are changed in the pathological state of diabetes.

OBJECTIVE@#To investigate the anti-leukemia effect of total saponins of Rubus parvifolius L. (TSRP) on K562 cell xenografts in nude mice and the mechanisms of action.@*METHODS@#The K562 cell xenografts in nude mice were established, and then randomly divided into 5 groups, the control group, the cytosine arabinoside group(Ara-c) and 3 TSRP groups (20, 40 and 100 mg/kg). The tumor volume and mass of each group of nude mice were measured and the anti-tumor rates of TSRP were calculated subsequently. The apoptosis status of tumor cells was detected by hematoxylin-eosin (HE) and terminal dexynucleotidyl transferase (TdT)-mediated dUTP nick end labeling (TUNEL) staining analysis. Finally, the activities of apoptosis related signaling of signal transducer and activator of transcription 3 (STAT3), eukaryotic initiation factor 4E (eIF4E) and B-cell lymphoma-2 (bcl-2) were determined with immunohistochemistry tests.@*RESULTS@#Subcutaneous injection of K562 cells induced tumor formation in nude mice, and the TSRP treated group showed a signifificant inhibitory effect on tumor formation. The nude mice treated with TSRP showed a signifificant decrease in tumor growth rate and tumor weight in comparison to the control group (all P<0.05). The HE staining and TUNEL assay showed that TSRP induced cell death by apoptosis. The immunohistochemical assay showed down-regulation of the bcl-2 gene in the TSRP treated cells. The phosphorylation levels of eIF4E and STAT3 were decreased obviously after the treatment of TSRP.@*CONCLUSION@#TSRP had an excellent tumor-suppressing effect on K562 cells in the nude mice xenograft model, suggesting that TSPR can be developed as a promising anti-chronic myeloide leukemia drug.

ABSTRACT

Due to their similarities with humans in anatomy, physiology, and genetics miniature pigs are becoming an attractive model for biomedical research. We aim to establish and evaluate blood type O cells derived from Korean native pig (KNP), a typical miniature pig breed in Korea. Ten cell lines derived from 8 KNP piglets and one adult female KNP (kidney and ear tissues) were established. To confirm the presence of blood type O, genomic DNA, fucosyltransferase (FUT) expression, and immunofluorescence staining were examined. Additionally, fluorescence-activated cell sorting and somatic cell nuclear transfer were performed to investigate the normality of the cell lines and to evaluate their effectiveness in embryo development. We found no significant bands corresponding to specific blood group A, and no increase in FUT expression in cell lines derived from piglets No. 1, No. 4, No. 5, No. 8, and the adult female KNP; moreover, they showed normal levels of expression of α 1,3-galactosyltransferase and cytidine monophosphate-N-acetylneuraminic acid hydroxylase. There was no significant difference in embryo development between skin and kidney fibroblasts derived from the blood type O KNPs. In conclusion, we successfully established blood type O KNP cell lines, which may serve as a useful model in xenotransplantation research.

ABSTRACT

Objective: To investigate the effects of preoperative sarcopenia on postoperative clinical outcome in patients with gastric cancer. Methods: A prospective study was performed in 93 patients with gastric cancer and the skeletal muscle mass was examined by bioelectrical impedance. The primary outcome was postoperative complications. The secondary outcomes were postoperative length of stay, overall hospital costs, 60 days re-admission and mortality rate. Results: A significant difference (sarcopenia group vs. non-sarcopenia group) was observed in the rates of overall postoperative complications, incidence of pleural effusion and intra-abdominal infection, overall hospital stay and postoperative hospital stay (P < 0. 05). The rates of 60 days readmission, 60 days mortality and hospital costs in sarcopenia group were not different between the two groups (P > 0. 05). By univariate and multivariate analysis, preoperative sarcopenia and preoperative nutritional risk are the risk factors of postoperative complications. Conclusion: Sarcopenia is an independent risk factor for postoperative complications in patients with gastric cancer; preoperative patients with sarcopenia have a worse prognosis and should be screened and optimized before surgery.

ABSTRACT

Evidence-based medicine (EBM) requires integration of available ‘best’ clinical evidence,clinician's judgment as well as patient's wishes and expectations.In this paper,we addressed the EBM framework for clinical decision making clinical practice in cardiovascular diseases.Evidence evaluated and interpreted are crucial tools for EBM.EBM has had a profound impact on the treatment of cardiovascular diseases,and confirmed the efficacy of certain controversial interventions.On the other hand,certain widely used conventional strategies for cardiovascular diseases had been proved to be ineffective.Yet,the scientific evidence from China is under resourced to impact the ability to achieve effective actions.We suggest to carry out more high-quality randomized clinical trials to support effective EBM clinical practice.

ABSTRACT

Objective To investigate the expression levels of miR-494 in breast cancer tissues and cell lines,and its correlations with the clinical stages,metastasis and prognosis of breast cancer.Methods The expression levels of miR--494 in 5 breast cancer cell lines,including MDA-MB-231,MDA-MB-453,HCC-1937,MDA-MB-468 and MCF-7,and normal breast cell line HBL-100,were detected by quantitative real-time polymerase chain reaction (qRT-PCR).The breast cancer tissues and normal adjacent tissues from 54 patients with breast cancer were collected,and their expression levels of miR-494 were determined by qRT-PCR.Then,the correlations of miR-494 levels with clinical stages,metastasis and prognosis of breast cancer were analyzed.Results The expression levels of miR-494 in MDA-MB-231,MDA-MB-453,HCC-1937,MDA-MB-468 and MCF-7 cells were significantly lower than that in HBL-100 cells (t =212.9,37.73,27.53,10.61 and 19.46,respectively,and all P <0.05).The expression levels of miR-494 in breast cancer tissues were also significantly lower than that in normal adjacent tissues (t =5.80,P < 0.01).Moreover,the expression of miR-494 was significantly related to the clinical stage (x2 =17.41,P <0.05),tumor grade (x2 =5.33,P <0.05),C-erbB-2 expression (x2 =9.83,P < 0.05) and the percentage of Ki-67 positive cells ((2 =6.13,P < 0.05) of breast cancer.Conclusion The expression levels of miR-494 are significantly dowrregulated in breast cancer tissues and cell lines,and related to the clinical stage,metastasis and prognosis of breast cancer,indicating that miR-494 may serve as a new biomarker for the diagnosis and prognosis of breast cancer.

ABSTRACT

Herein, we describe a case of uterine calcification in the uterus of a pig without pregnancy loss. The recipient underwent cloned embryo transfer and Cesarean section for safe delivery of cloned piglets. During the Cesarean section, 4 white, star-like, (2 × 2 × 2) cm, calcified structures were found within the endometrial cavity. Despite dystrophic calcification around the placenta, healthy cloned piglets were produced successfully. To our knowledge, this is the first reported case of dystrophic calcification occurring within the uterus in a pregnant pig.

ABSTRACT

<p><b>OBJECTIVE</b>To determine the antiproliferative activity of Rubus parvifolius L. (RP) extract, its medicinal serum and RP total saponins (RPTS) against K562 cells in vitro and in vivo.</p><p><b>METHODS</b>Nude mice models bearing leukemia tumors were treated with different concentrations of RP extract. The size, weight and histopathological change of leukemic tumors were determined. Semi-solid agar culture and methylthiazolyl tetrazolium (MTT) assay were used to determine in vitro the inhibition of colony formation and proliferation of K562 cells respectively by different concentrations of RP medicinal serum and RPTS.</p><p><b>RESULTS</b>RP extract had a tumor inhibition rate of 84.8% when administered to mice at a dose of 1.0 g/day of crude RP root equivalent. Semi-solid agar culture of K562 cells in the presence of 20% (v/v) of RP medicinal serum and 150 mg/L RPTS demonstrated a 50.8% and 100% inhibition of the colony forming unit (CFU)-K562, respectively. The same doses of RP medicinal serum and RPTS showed a proliferation inhibition of 31.4% and 86.3%, respectively against K562 cells in MTT assay.</p><p><b>CONCLUSION</b>RP extract and RPTS show effective antiproliferative activity against myeloid leukemia cells in vitro and in vivo.</p>

ABSTRACT

Objective To study a quantitative indicator for measuring the similarity between blood pressure waveforms and its application in the analysis on the simulated blood pressure waveform for pulsatile flow simulation system.Methods Based on the past similarity measurement algorithm and the known feature of blood pressure waveforms, the weighted average algorithm was presented in this paper, which possessed advantages of both global matching and partial matching by integrating the correlation coefficient with the characteristic parameter algorithm, and calculating similarity degrees through overall and partial waveform.Results The weighted average algorithm was proved to be more suitable for the analysis on the blood pressure waveform similarity compared with the angle cosine method, average absolute deviation method, and numeric similarity coefficient method.Conclusions The weighted average algorithm showed excellent ability in calculating the similarity degree between different waveforms, or in comparing the performance with different pulsatile flow simulation systems, and it could be applied to other physiological waveforms with further improvement.

ABSTRACT

<p><b>OBJECTIVE</b>To develop a drug delivery system triptolide-polyethylenimine-cyclodextrin and to evaluate its anticancer activity in vitro.</p><p><b>METHODS</b>Triptolide was conjugated to polyethylenimine-cyclodextrin by N, N'-carbonyldiimidazole to form triptolide-polyethylenimine-cyclodextrin. (1)H-NMR, FT-IR and XRD were used to confirm its structure. The anticancer effect of the polymer was assessed by MTT assay, erasion trace test and hematoxylin-eosin staining. The potential to condense siRNA and to delivery siRNA into cytoplasm was demonstrated by gel retardation assay, zeta-potential determination and fluorescence staining.</p><p><b>RESULTS</b>Triptolide was successfully conjugated to polyethylenimine-cyclodextrin and the conjugation rate of triptolide was 10% (w/w). siRNA was effectively condensed by the polymer at the N/P ratio of 5, and its particle size was 300 ±15 nm and zeta potential was 8 ±2.5 mV. MTT assay, erasion trace test and hematoxylin-eosin staining revealed that triptolide-polyethylenimine-cyclodextrin had anticancer effect and low cytotoxicity to normal cells. The polymer was able to deliver siRNA to the cytoplasm effectively as demonstrated by fluorescence staining.</p><p><b>CONCLUSION</b>Triptolide-polyethylenimine-cyclodextrin is able to inhibit the growth and migration of cancer cells in vitro and to carry siRNA into cells effectively. It is potential to be used as a novel prodrug for co-delivery of gene and drug in cancer treatment.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the characteristics of cationic polymers polyethylenimine-β-cyclodextrin (PEI-CyD), polyethylenimine-poly-(3-hydroxypropyl)-aspartamide (PEI-PHPA), N,N-Dimethyldipropylenetriamine-Bis(3-aminopropyl)amine-aspartamide (PEE-PHPA) in vitro and in vivo.</p><p><b>METHODS</b>PEI-PHPA, PEI-CyD and PEE-PHPA were synthesized and the chemistry structure of PEI-PHPA, PEI-CyD and PEE-PHPA was confirmed by (1)H-NMR. The particle size and zeta potential of these polymers were measured, and capacity of plasmid DNA condensation was tested. The inhibition of COS-7, A549, HEK293 and C6 cells was measured by MTT assay. The transfection efficiency was determined in HEK293 cell lines. The toxicity, tissue distribution and transfection efficiency of cationic polymers were tested in vivo.</p><p><b>RESULTS</b>When the N/P of polymers/DNA at 30, the particle sizes were close 250 nm and the zeta-potential were near 35 mv. They were able to condense DNA at N/P ratio < 5. The MTT assay showed that the IC(50) of PEE-PHPA was 21.5, 20.2, 7.30 and 37.1 μg/ml, and that of PEI25kD was 15.8, 18.3, 11.4 and 36.7 μg/ml in C6, COS-7, A549 and HEK293cell lines, respectively. The cell viability of PEI-CyD and PEI-PHPA in above cell lines was over 60%. They had high transfection efficiency in HEK293 cell lines. The LD(50) of PEI25Kd, PEI-CyD, PEI-PHPA and PEE-PHPA in vivo was 19.50, 100.4, 521.2 and 630.0, respectively by intraperitoneal (ip) injection. The contractions of these polymers were higher in kidney than in other organs and tissues.PEE-PHPA had slight effect on kidney and liver function.</p><p><b>CONCLUSION</b>PEE and PEI25kD have higher transfection efficiency and higher toxicity; while PC and PHPA-PEI have lower toxicity and higher transfection efficiency to be used as non-viral gene vector.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To develop polyethylenimine-Doxorubicin-montmorillonite (PEI-Dox-MTT) as a novel multifunction delivery system.</p><p><b>METHODS</b>Dox was intercalated into montmorillonite, PEI covered to the surface of Dox/MMT to make the nano-particle. XRD, FT-IR and TGA were used to confirm chemical property of the nano-particle. SEM was used to observe the morphology. The capability of drug release was investigated by PBS buffer solution (pH 7.4). The DNA binding ability of nano-particle was detected by gel electrophoresis retardation assay. The cell viability in COS-7 and SKOV3 cell lines was tested using MTT assay. The gastric mucosa protection was evaluated in vitro.</p><p><b>RESULTS</b>XRD image showed that Dox was intercalated into montmorillonite, inter space of which increased to 31.3Å; the FT-IR spectra showed the vibration bands of PEI at 1 560 cm(-1) and 2 850 cm(-1), the vibration band of Dox at 1 350 cm(-1). Size analysis and SEM revealed that the size of nano-particle was 600 nm, and the zeta-potential was 30 mV. Drug release experiment explored that the nano-particle stably released drug in range of 6 X10(-4) ≊ 8 X10(-4) mg/ml within 72 h. MTT assay showed that the cell viability was over 80% in experiment condition in COS-7 and SKOV3 cell lines. 0.3 mg PEI-MMT nano-particle was able to protect gastric mucosa from alcohol.</p><p><b>CONCLUSION</b>Multifunction system of PEI/Dox/MMT has been prepared successfully.</p>

ABSTRACT

Objective To study the flow field of a new vascular bioreactor with small diameter at different operating conditions. Method To investigate the flow field of a new vascular bioreactor with small diameter under three operating conditions by numerical simulation: outer box rotating, inner box rotating and both inner box and outer box rotating. A comparison of parameters such as flow velocity and shear stress was made. Results The culture medium in the bioreactor rotated with the inner box and outer box, and the velocity was uniformly distributed; the bioreactor could therefore provide a low shear stress environment that is nondestructive for the cells in the medium; the shear stress in bioreactor had a quadratic function relation with the rotation rate. Conclusions This bioreactor could provide a good culture environment for vascular tissue regeneration, and this study provided theoretical basis for the experimental research of bioreactors.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the chemical composition of the modified surface of fluoride ion-implanted titanium and assess the effect on the formation of focal adhesion plaque in vitro.</p><p><b>METHODS</b>Pure commercial titanium discs were treated with fluoride ion implantation by plasma immersion ion implantation technique (PIII) and chemical composition and value of the surface modification layer were characterized by X-ray photoelectron spectrometer (XPS). In order to investigate the formation of focal adhesion plaque, MG-63 cells were seeded onto the surfaces of the modified Ti discs and quantified by morphometric analysis using an immunofluorescence microscope.</p><p><b>RESULTS</b>The full range XPS spectra and fitting results indicated that the surface of fluoride ion-implanted titanium was the mixture of titanium dioxide and titanium trifluoride. Meanwhile, the quantity of focal adhesion plaque on fluoride ion-implanted titanium was more than that on the non-implanted titanium after 6 hours' cell culture.</p><p><b>CONCLUSION</b>The XPS data revealed that the modified surface layer of fluoride ion-implanted titanium contained titanium dioxide and titanium trifluoride, which could enhance the formation of focal adhesion plaque.</p>
